COPD複雜的挑戰

在2015年12月Lancet Respiratory Medicine中發表的一篇文章,文章的標題是”The complex challenge of COPD”。 [1] 在2015年中有許多的文章發表,不約而同的認定COPD是一個heterogeneous疾病。文章中提到幾個重要的挑戰,在此分別跟讀者說明。
第一個是雖然抽菸是導致COPD主要的原因而引發肺功能下降的狀況。但是更主要的原因可能包括環境污染、老化和反覆性的發炎和感染導致的變化。在三個大規模的世代研究中(the Framingham Offspring Cohort;the Copenhagen City Heart Study;and the Lovelace Smokers Cohort) [2]發現不管抽菸與否,如果在40歲以前有較差的肺功能(FEV1<80% of predicted),而且肺功能退化速度正常,但是將來得到COPD的機會是比較高的。所以並不是肺功能的加速惡化模式才會得到COPD。也再次說明環境的影響和基因的易感性都會與得到COPD有關。
第二個是基因研究時代的來臨與衝擊。在Wain 等學者 [3] 在英國Biobank的資料庫分析中發現有特定的基因(KANSL1, TSEN54, TET2, and RBM19/TBX5)與罹患COPD有關,不管是否有吸菸行為。抽菸影響基因變化和COPD發生相關性只佔少數族群。再一次印證COPD發生可能包括基因變化,與抽菸導致的機轉是互相獨立的事情。因此基因的獨特性、環境的因素、和個人的生活習性都是得到COPD重要的成因。
第三個可以透過了解基因的變化讓用藥更為精準和正確。在一個RCT的研究中探討ICS對於COPD治療的反應(GLUCOLD trial) [4-7]。研究發現如果在氣管表皮細胞表現出Th2-cell 相關的基因訊息時(asthma-related genetic signatures, as determined by the T-helper-2 (Th2) signature score (T2S)),那使用上ICS的效果會比沒有表現T2S 基因訊息的病患更好。我們知道Th2-cell 相關的基因訊息會啟動一系列像氣喘反應的發炎變化例如: increased airway wall eosinophil counts, increased percentage of blood eosinophil, and bronchodilator reversibility。這研究證實在COPD的患者在基因表現的情形出現了像氣喘發炎的Th2-cell mediated inflammation,而這一類的病患將會使用上ICS的藥物治療得到的效果更加顯著。
第四個是未來有許多新的藥物出現希望能減少COPD病患急性發作的次數。在一個研究中發現在病患使用三合一的藥物(ICS+LABA+LAMA)治療下仍常急性發作需要住院的病患,加上Roflumilast 會明顯減少發作次數(REACT study) [8]。同樣的,對於這些常惡化的病患我們應給予更積極有效的治療策略,否則對於醫療負擔及病患預後都帶來嚴重的後果。
最後,個人想引用原作者的話來闡述我們現今面對COPD的挑戰與未來的方向,提出給大家思考: Studies of biomarkers, molecular phenotyping, improved imaging technologies, and advances in genetic research modalities improve our understanding of the natural history of COPD. Through these and other advances, we continue to learn about the heterogeneity that exists in disease phenotypes and the natural history of COPD. These findings, combined with a series of new drug treatments coming to the market, herald exciting times in the care of patients with COPD.
